Social workers are vital members of the Care Plus Hospice team. They provide education and emotional support when referrals are made and throughout the duration of hospice services. Patients and especially their loved ones highly value social workers. They help patients and their loved ones make sense of advanced directive documents, including “Do Not Resuscitate” orders and living wills. Care Plus Hospice social workers often facilitate conversations between family members who may not always see eye to eye during this stressful time. They meet patients and family members in the home setting. Social workers are adept at problem-solving when family members live at a distance. They meet with medical staff and nurses regularly to ensure client dignity and empowerment. Social workers are very knowledgeable about community resources and can be of assistance if higher levels of care are warranted.
